Club renewal forms must be turned in to Amy Sims, room H-1015B by September 24, 2009 to be chartered or renewed in October.
Forms
These forms give OSA current information regarding the club and its members and help us to coordinate the monthly calendar of events.
Copies of the forms are also available in the appendix of the Survival Guide (PDF, 1MB), which can also be found in the OSA office on the bridge.
It is important for existing clubs to raise enough money to allow them to function until new allocated money is available after chartering in October.

Please Note: At the October meeting, all clubs renewing their charter will be listed on a sheet of paper to be voted on collectively. This will expedite the chartering process. The chartering will be delayed until a member is present.
