Form Instructions
- Fill out appropriate information and return to Media Services.
- Media Services will then create the business card artwork and return a sample copy attached to the business card form.
- Review the sample artwork and verify accuracy, if not correct indicate changes on sample artwork and return to Media Services, if correct proceed on to number 4.
- If the artwork is correct, please sign, date and provide requested billing information.
